	Any customer getting a body piercing, including minors, must have government issued Identification (passport, state identification card, drivers license, permenant resident card etc.) This must be original unexpired Identification, not a copy. Minors (anyone under 18 years of age) Must be accompanied by their parent/legal guardian. Both the parent/legal guardian and minor, must have a recent photo ID. If the minors ID is a school photoID card, you must present a birth certificate as well. If the parents last name is differant from the minors, we need documentation to show why. This would be divorce papers, custody papers, marriage certificate, etc. If the parent is listed on the birth certificate of the minor, including the birthdate of the parent, this would serve as proof.
